Replace D3.js with Chart.js, add mailing list form
This release improves state-of-the-art charts aesthetics, adds a mailing list subscription form, and fixes various other small aesthetic issues, like link highlighting.

This release is a combination of further "reskinning" changes and small bug fixes.
Partial re-skinning
A partial UI re-skinning has been applied to the look-and-feel of the app. (More UI/UX work will follow.)
Remove network map
Per Unitary Fund discussions, the tasks index network map did not seem to add value. Pending its replacement, it has been removed.
Results "notes" field
This release adds a "notes" fields to "results" table rows for adding any miscellaneous context to a new result entry, which can immediately be viewed by all users.
- Additionally, description editing for tasks and methods now populates the existing text in the editing modal.
- Also, state-of-the-art charts render for as few as 3 data points, instead of 4.
(Since database schema has changed, as well as API schema, this motivates a semantic "major version" change to v0.2.)
Child tasks in task detail view
This release adds a table of any "child" tasks to all corresponding task detail views, for easier navigation.
